Carrying 6th rescue down an escape, his axe became caught in the wire rope as the canvas chute caught fire. The woman he was carrying fell but was unhurt, he though fell to the ground on fire, after freeing himself. Considerable criticsm of the of the design of the Escape Ladder and its chute was made by the coroner. Recorded in Postman's Park, London.
